Instructions for Completing Your New Hire Packet: 
  1. Payroll Setup:
    Please begin by reading the instructions provided by Janssen Accounting, our payroll company.

  2. Employee Information Form:
    Complete the Employee Form in full and submit it as directed.

  3. Tax & Employment Forms:
    In this section, you’ll find downloadable versions of the W-4, WT-4, and I-9 forms. Please follow the instructions for each form. Multiple submission options are available.

  4. Direct Deposit Form:
    Complete the Direct Deposit form located below the payroll instructions. Be sure to follow the steps outlined at the end of that section carefully.

  5. Need Help?
    If you have any questions at any point, feel free to reach out to Matt or Sarah for assistance.

How to Complete W-4, WT-4, and I-9 Forms Without Adobe Acrobat Pro or DocuSign

You do not need Adobe Acrobat Pro, DocuSign, or any paid software to fill out these forms.

Step-by-Step Instructions: (Paid Signature Software is Not Needed)


For W-4 Form: Employee’s Withholding Certificate

How to Fill It Out Without Software:

  1. Download the combined PDF file to your device.

  2. Open the file using one of these free tools:

    • Google Chrome or Microsoft Edge (Recommended)

      • Right-click the PDF file → Select Open with → Choose Chrome or Edge

    • Mac Preview (Mac users only)

      • Double-click the file to open in Preview

    • Online PDF Editors:

  3. Complete all required fields by typing directly into the fillable fields.

  4. Save the completed W-4:

    • In Chrome or Edge: Click the printer icon → Choose "Save as PDF"

    • In Mac Preview: Go to File > Export as PDF

  5. Sign the form digitally.

  6. Submit using the button at the end of this section.

For the WT-4 Form:

How to Fill It Out Without Software:

  1. Use the same combined PDF file from earlier.

  2. Navigate to the WT-4 Form and complete ONLY Section 1 – Employee Section).

  3. Complete Section 1 only:

    • Enter your personal details (name, address, date of birth, citizenship status, etc.)

    • Do not fill out Section 2 – this is for employer use only.

  4. Sign the form (at the end of Section 1):

    • Sign the form digitally. We allow digital typing of your name as a signature.

  5. Save or export the completed form as a PDF using your browser or PDF editor.

For the I-9 Forms: Employment Eligibility Verification

How to Fill It Out Without Software:

  1. Use the same combined PDF file from earlier.

  2. Navigate to the I-9 Form (Section 1 – Employee Information and Attestation).

  3. Complete Section 1 only:

    • Enter your personal details (name, address, date of birth, citizenship status, etc.)

    • Do not fill out Section 2 – this is for employer use only.

  4. Sign the form:

    • Sign the form digitally. We allow digital typing of your name as a signature.

  5. Save or export the completed form as a PDF using your browser or PDF editor.

Submission Options:

  1. Hand Deliver – Coordinate an in-person meeting with Matt or Sarah.

Employee Direct Deposit Authorization Form

I authorize my employer and / or their payroll service, to directly deposit my net payroll check into the bank account that I specify below.


I understand that funds will post in my bank account on the specified payroll basis, unless that day falls on a federal banking holiday. If that day does fall on a federal banking holiday, my funds will be deposited on the next business day.


In the event of a system or banking error during the electronic transfer of my paycheck. I authorize my employer and / or their payroll service to correct that error by reversing out (withdrawing) the error made from my account and as soon as possible depositing the correction into my account.


NOTE: A mistake in hours missing on your timecard / paycheck is not a reason for the reversal of funds. Those kind of errors will simply be corrected on the next payroll.

Please select type of account you want your net payroll check direct deposited into. If you are having your check deposited into multiple accounts, please provide the information requested below for each bank and account. You will also need to provide the dollar amount or the % you want in each account.

Handwritten notes or Deposit slips are not valid documentation.
